Thank you for trusting us with your health care. The 374th Medical Group is a certified Primary Care Medical Home. Your PCMH team consists of doctors, providers, nurses, technicians, and support staff from one of the following assigned primary care clinics:

PCMH certification is earned through continuous surveys conducted by The Joint Commission, the nation's largest accrediting body for over 22,000 medical facilities across the country. The certification focuses on keeping the patient engaged at the center of health care and the driver of care rather than as a passive recipient.

What is a Primary Care Medical Home?

Five PCMH core attributes directly impact the care you receive at the 374th MDG:

Patient-Centered Care

Your medical home is a place of care where patients and family members are treated with respect, dignity, and compassion. This allows for a strong and trusting relationship between our patients and their care team.

Comprehensive Care

You're assigned to a PCMH team consisting of your primary care manager (physician, nurse practitioner, or physician assistant), a nurse, medical technicians, and an administrative technician who partners with you to help provide all your health care needs.

Our clinic cares for Department of Defense beneficiaries enrolled in TRICARE Prime and Plus that are active duty service members, retirees, and their family members. Our PCMH teams serve in five co-located primary care clinics consisting of the Family.

Each of these PCMH clinics offer comprehensive, outpatient primary care, to include acute care, preventive care, routine physicals, disease management, women's health exams, and minor procedures.

Superb Access to Care

Our clinic is open Monday-Friday, 7:30 a.m. to 4:30 p.m., except on federal holidays. You can conveniently schedule an appointment with a call to the 374th MDG Appointment Line from a US number at 011-81-42-552-2510, extension 58864, JP number at 042-552-2510, extension 58864, or DSN at 315-225-8864, during normal clinic hours to speak to our friendly staff.

You can readily access care for non-emergent healthcare needs that arise after-hours or while traveling out of the local area by contacting the Military Health System Nurse Advice Line by dialing 800-TRICARE (874-2273) or by visiting the MHS Nurse Advice Line site. The MHS Nurse Advice Line is available 24/7 by phone, web chat, and video chat.

Finally, the 374th MDG provides access to several services that add value to every visit you make, such as pharmacy, immunizations, laboratory, and radiology. The pharmacy offers the most affordable option for beneficiaries to get their medications. The Immunizations Clinic provides a wide range of routine and special vaccines (e.g., for deployers) to help prevent infectious diseases. Radiology provides common radiographic exams (X-rays). This saves you time by not having to travel to different facilities. It also gets the information back to your PCMH team quickly so they can guide your treatment.

What to do in an emergency

When on base call 911 from a DSN phone, 011-81-42-552-2510, extension 911 from a US number or 042-552-2510, extension 911 from a JP number, The 374th MDG has an Urgent Care and Emergency Medical Services that respond to on base emergencies.

Systems-Based Approach to Quality and Safety

Our electronic health record, MHS GENESIS, is constantly undergoing modernization to empower our staff's ability to leverage advanced health informatics. These updates implement clinical decision support tools that guide recommendations tailored to individual patients' specific needs.

Coordinated Care

As a patient, you're an active participant in managing your health. Our shared goal is to keep you as healthy as possible. Your PCMH team helps you coordinate all aspects of your care, to include wellness and acute visits, referrals for specialty care, obtaining results, and consultations with other providers for second opinions. Our Medical Management Team provides compassionate case management and disease management and has specialized knowledge of preventive health measures, outreach programs, and the Exceptional Family Member Program.

Choosing Your PCMH Team

374th MDG patients may change their PCM by visiting our TRICARE Operations and Patient Administration (TOPA) office or by visiting the milConnect website. Active duty patients are assigned according to their unit for operational reasons, and are only switched in rare, extenuating circumstances

Communication is Key

To receive the most benefit from time spent at the 374th MDG, we suggest the following important tips to ensure you can communicate with your health care team:

Sign up for the MHS GENESIS Patient Portal. The portal enables you to view your health information, send messages to your PCMH team, request prescription renewals, view clinical notes, look up results, and review past and upcoming appointments. You can find a wealth of information on our website W. about how to contact specific offices and obtain services throughout the 374th MDG. Finally, you can visit our Facebook  to get updates about what's going on at the 374th MDG, such as unexpected closures and special events.

Bring all your medications to your appointment (preferably in the original containers) and a list of any over-the-counter medications and supplements you're taking and describe when, how, and how often you take each of your medications and supplements.

Write down any questions and notes you may have in advance. Feel free to bring a pen and paper to your appointments. If necessary, bring a friend or family member to your visit to help you remember answers to your questions or other details of the visit. Here are some questions to ask every time you talk with a doctor/provider, nurse, or pharmacist: What's my main problem? What do I need to do? And why is it important for me to do this?

Provide a complete and honest description of your situation and concerns, even if it is difficult or potentially embarrassing to talk about. Let the team know if you have a current care plan and if you feel that you're making progress towards your goals, or if you need further help or information.

Speak Up

Our clinic staff respects and encourages patients and family members to speak up! Our job is to make sure you're comfortable expressing your preferences, needs, and values. Your responsibility is to convey them, as well as your concerns and questions. If you don't fully understand, ask for further clarification. Your team should always use simple terms that are easy to understand and in a language you prefer.

Take Charge of Your Health - Be an Informed Health Consumer

If you have a chronic condition, learn more about your disease, treatment options, treatment plans, and prognosis. Help us identify what your goals are and how we can help you reach those goals. We're partners in your care; you are an active partner. We create plans of care with you, not for you. If you seek online information on your condition, only use reputable websites recommended by your PCMH team to avoid anxiety, confusion, and possibly, costly scams.
